  • Opening Balance Equity changes without a clear reason
  • Bank or card balances do not match statements
  • Income appears twice through invoices and deposits
  • Old money remains in Undeposited Funds
  • Accounts receivable or payable contain years-old balances
  • Sales-tax or payroll liabilities disagree with filed reports
  • Personal spending is mixed into business categories
  • Accounts were deleted and recreated to force a balance
